Position: Advanced Health Worker, Aged Care Service, Community Health Services
Work Location: Rockhampton region

To assess older persons within the Aged Care Assessment Program (ACAP) guidelines for eligibility to access Commonwealth subsidised services and other community services. Actively contribute to the planning and delivery of aged care initiatives aimed at improving access to aged care services for Aboriginal and Torres Strait Islander people.

You will be assessed on your ability to demonstrate the following key attributes. Within the context of the responsibilities described above, the ideal applicant will be someone who can demonstrate the following:

  1. Excellent communication skills with clients, service providers and co-workers at all levels.
  2. Moderate competence with computers and data entry.
  3. Able to work autonomously and remain within your scope of practice.
  4. Possesses a quality client and aged care focus.
  5. Positive team player with a good work ethic, and ability to maintain a busy work load.

Qualifications/Professional registration/Other requirements:
  1. Diploma in Aboriginal and/or Torres Strait Islander Primary Health Care is mandatory for this position.
  2. Applicants in possession of a Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (or equivalent) and working towards a Diploma level may be considered for this position, and if successful, appointed to 004 level on a temporary basis until proof of achievement of Diploma level can be provided.
  3. Applicants who have the required Diploma qualification must take priority over applicants who have not yet reached this qualification.
  4. This position requires the incumbent to operate a C Class motor vehicle, and an appropriate licence endorsement to operate this type of vehicle is required. Proof of this endorsement must be provided before commencement of duty.
  5. Potential applicants are advised that the Aged Care Act 1997 requires Queensland Health employees and volunteers of aged care services to have a current National Police Certificate. Queensland Health will facilitate the applicants obtaining the above check.
  6. Under s25 of the Anti-Discrimination Act 1991 (Qld), it is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander community.
  7. Appointment to this position requires proof of qualification and registration or membership with the appropriate registration authority or association. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
